Chronic Disease Management
The roll-out of a new Chronic Disease Management Programme for GMS/GPVC patients has started.
The first phase of the Programme is targeting medical card & Doctor Visit card patients, aged over 70 years, with the following chronic diseases:
- Diabetes Type 2
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Cardiovascular Disease including:
- Heart Failure
- Ischaemic Heart Disease
- Cerebrovascular Disease (Stroke/ Transient Ischemic Attack (TIA))
- Atrial Fibrillation
Patients who are entitled to this new HSE initiative will be contacted by our practice team to attend the surgery in person for a nurse consultation or remotely by telephone in line with current Covid19 restrictions.
Patients will then be contacted in six months’ time after their nurse consultation to attend their doctor for a follow up GP consultation. These reviews will include patient education, preventative care, medication review, physical examination, investigations and an individual care plan.
